29 CFR 1910.26(c)(3)(i): The angle of the ladder was not proper in that distance from the base to the vertical wall was not equal to one-fourth the working length of the ladder: On or about August 26, 2015, at the above addressed jobsite, an employee was using an aluminum/fiberglass extension ladder to change-out three electrical switches at a sub-station, wherein the ladder was not set-up at the proper angle (4:1 Ratio, 25 Percent or 14 Degrees); and wherein the employee on the ladder was working within close proximity to the energized adjacent overhead lines; therefore exposing the employee to fall and electrical hazards.
