The employer did not institute a training program and ensure employee participation in the program: a) Barge cleaning area; on or about August 27, 2015 the employer failed to implement a Hearing Conservation program to train the employees who were exposed to noise levels at or above an 8-hour time-weighted average of 85 decibels while cleaning closed top barges.
