29 CFR 1910.179(n)(3)(xi): When starting the bridge and when the load or hook approaches near or over personnel, the warning signal shall be sounded. On or about August 18, 2015, and at times prior thereto, during maintenance procedures at National Gypsum Company dba NGC Industries, LLC., 4189 Hunt Street, Pryor, OK 74361, the employer did not ensure that when starting the bridge and when the load hook approaches near or over personnel the warning signal is sounded. The employee was exposed to a struck-by hazard.
