On or about, August 25, 2015, and at times prior to, at the Lubrizol Corporation in Deer Park, TX, the employer did not implement a Management of Change (MOC) in the 105 Control Room of the 116 Unit when the Trailer Mount York 20 Ton portable HVAC system replaced the control room's existing system that was in need of repair.

On or about, August 25, 2015, and at times prior to, at the Lubrizol Corporation in Deer Park, TX, the employer did not ensure that employees working in 105 Control Room of the 116 Unit were informed and trained on the temporary operation of the Trailer Mount York 20 Ton portable HVAC system when the control room's existing system needed repair.
