Instance B) One employee, working below the employees in instance A, was exposed to falling materials, equipment and debri without using head and eye protection.

29 CFR 1926.452(j)(2): Poles of pump jack scaffold(s) were not secured to the structure by rigid triangular bracing, or equivalent, at the bottom, top and other points as necessary: Location: 28 W Broadway, So Boston MA: The pump jack scaffold poles were not braced at their base, exposing the two employees on the scaffold to a fall as well as a struck by hazard to the one employee working under and around the scaffold.
