29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, were not used for access: JOB SITE: 2601 Ohio Street, Saint Louis, MO 63118, on or about 8/26/2015, workers were observed climbing over a ladder jack scaffold platform, approximately 12 feet 3 inches above ground level without portable ladders, hook-on ladders, or attachable ladders. The workers were exposed to fall hazard.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: JOB SITE: 2601 Ohio Street, Saint Louis, MO 63118, on or about 8/26/2015, workers were observed working from a ladder jack scaffold, approximately 12 feet 3 inches above ground level, actively doing tuck-pointing work and were not protected by a personal fall arrest system. The workers were exposed to a fall hazard. The LUCAS MASONRY RESTORATION was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 974269, citation number 1 , item number 1, issued on 8/18/2014, and was affirmed as a final order on 9/8/2014, with respect to a workplace located at 6610 waterman Avenue, Saint Louis, MO 63130.
