29 CFR 1910.23(d)(1)(iii): The employer did not ensure that standard stair railings were provided on the open sides of stairways less than 44 inches wide: (a) Loading Dock: Employees use a fixed metal staircase 36 inches wide, equipped with four risers to gain access to the loading dock. The stairway was not equipped with standard railings, in that it did not have an intermediate rail on the open sides of the stairway as determined on August 25, 2015.

29 CFR 1910.23(d)(1)(iv): The employer did not ensure that standard stair railings were provided on the open sides of stairways more than 44 inches but less than 88 inches: (a) Maintenance Area Outside: Employees use a fixed concrete staircase 48 inches wide, equipped with five risers to gain access to the maintenance area from outside the building. The stairway was not equipped with a standard railings, in that it did not have an intermediate rails on the open sides of the stairway as determined on August 25, 2015.
