29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 27 August 2015, at the above addressed jobsite two employees were working from an aerial lift installing vertical columns on newly constructed residence without the use of a body belt and lanyard while exposed to a 10-foot fall hazard.
