29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:  Employees were exposed to electrical hazards and the employer did not ensure the flexible cord and Porter Cable air compressor used to supply air for the pneumatic nail guns at the site were protected with a ground fault circuit interrupter.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  Employees were exposed to injuries from the scaffold shifting and the scaffold was placed on dirt and in landscaping rocks and suitable mud sills were not used.
