29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). Throughout Facility - On or about August 21, 2015, the employer failed to ensure that powered industrial truck operators were trained as required by the OSHA Standards, thereby exposing employees to serious struck by injuries. Lava SCS, LLC dba Logistics Team was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 610839, citation number 1, item number 1 and was affirmed as a final order on November 19, 2012, with respect to a workplace located at 2520 Haven Avenue, Joliet, Illinois 60433.
