(a) Park Avenue Dental @ 230 North Park Ave, Fort Lupton, CO 80621:  On or before August 25th 2015, the employer allowed employees to prepare food in the Laboratory where potentially infectious materials are present. This condition potentially exposed employees to foodborne biological hazards.  This condition was corrected during inspection when the microwave and other food preparation appliances were removed from the Laboratory.

Chemical-specific information must always be available through labels and material safety data sheets. If such hazards are present, or likely to be present, the employer shall:  (i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  (ii): Communicate selection decisions to each affected employee; and,  (iii): Select PPE that properly fits each affected employee.
