29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Worksite located at 8307 S Olympia Ave. Tulsa, OK 74132. On or about August 27, 2015, employees were observed performing work from an aerial lift without the use of a fall protection harness and lanyard. The employees were exposed to a fall hazard of more than 6 feet.
