29 CFR 1926.501(b)(3):   Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:  a)  71 Walsh Drive, On the Roof, Parsippany, NJ.  Workers were exposed to falls of 25 feet to the ground below as no methods of fall protection were being used while operating the swing hoist.                                                                    Violation occurred on or about 08/20/2015 and 08/21/2015.  b) 71 Walsh Drive, On the Southeast Section of the Roof, Parsippany, NJ.  Worker was exposed to falls of 25 feet to the ground below as no methods of fall protection were being used while operating the swing hoist.                                      Violation occurred on or about 08/22/2015.

29 CFR 1926.552(a)(1):   The employer did not comply with manufacturer's specifications and limitations applicable to the operation of all hoists and elevators:  a)  71 Walsh Drive, On the Southeast Section of the Roof, Parsippany, NJ.  Roofing worker was exposed to struck-by hazards as the Garlock Equipment Company's Swing Hoist safety precautions were not followed in that roofing materials or equipment were used as a counterweight for the hoist instead of using the 400 lb. permanent counterweight system.  In addition, hoisting was done by workers on the roof and the manufacturer specifies that rope handling be done from the ground.                                               Violation occurred on or about 08/22/2015.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  71 Walsh Drive, At the South End of the Building, Parsippany, NJ.    Roofing workers were exposed to falls of 25 feet to the ground below as the aluminum extension ladder used to access the roof did not have its side rails extending at least 3 feet above the roof's surface.                                    Violation occurred on or about 08/22/2015.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  undefined :     a)  71 Walsh Drive, On the Southeast Section of the Roof, Parsippany, NJ.    Roofing workers were exposed to falls of 25 feet to the ground below as methods of low-slope roof fall protection were not being used while gravel stops and metal coping were being installed.                                    Violation occurred on or about 08/22/2015.    S & S Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in OSHA inspection number 315555862, citation number 1, item number 1 and was affirmed as a final order on 08/29/2011, with respect to a workplace located at 45 South Main Street, Branford, CT.
