Roofing worker was exposed to struck-by hazards as the Garlock Equipment Company's Swing Hoist safety precautions were not followed in that roofing materials or equipment were used as a counterweight for the hoist instead of using the 400 lb. permanent counterweight system.  In addition, hoisting was done by workers on the roof and the manufacturer specifies that rope handling be done from the ground.                                               Violation occurred on or about 08/22/2015.
