At the rear of the house where an employee was cutting block with a gas powered saw, eye protection was not provided and required to be worn, thereby exposing employees to a struck-by hazard.

On the south/east side of the house where employees were working from a tubular welded frame scaffold, a screw jack was placed on a wooden mud sill that was deteriorated, exposing the employees to fall, struck-by, or other hazards.

29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units.   a. On the south/east side of the house where employees were working from a tubular welded frame scaffold, a screw jack was placed on a stack of 2x8s with one side of the stack supported with a piece of 2x4, exposing the employees to fall, struck-by, or other hazards.   b. On the south/east side of the house where employees were working from a tubular welded frame scaffold, a screw jack was placed on a 2x6 which was supported on one end with earth and the other across a cinder block, exposing the employees to fall, struck-by, or other hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a. On the scaffold where employees were installing stone to the south/east exterior of the house from a welded tubular frame scaffold, guardrails were not installed along all open sides and ends of platforms, thereby exposing employees to a 18 foot fall hazard.  b. On the scaffold where employees were installing stone to the north/east exterior of the house from a welded tubular frame scaffold, guardrails were not installed along all open sides and ends of platforms, thereby exposing employees to a 14 foot fall hazard.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.  a. On the first floor roof on the south/east side of the house where employees were working, the employees were not protected from falls by a guardrail system, safety net system, or personal fall arrest system, thereby exposing the employees to a 14 foot fall hazard.
