29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end.      a) Stairway Shaft - North: Employees were exposed to a fall hazards of approximately 30 feet while engaged in masonry activities from a welded frame scaffold with the platform extending over 2 feet over the support, on or about 8/29/15.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    a) Stairway Shaft - North: Employees were exposed to a fall hazards of approximately 30 feet while engaged in masonry activities from a welded frame scaffold that was not resting on base plates, on or about 8/29/15.  b) Elevator Shaft: Employees were exposed to a fall hazards  of approximately 30 feet while engaged in masonry activities from a welded frame scaffold that was not resting on base plates, on or about 8/22/15.  a) Stairway Shaft - South: Employees were exposed to a fall hazards  of approximately 30 feet while engaged in masonry activities from a welded frame scaffold that was not resting on base plates, on or about 8/8/15.
