29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  On or about August 27, 2015, at 5020 Cromwell Dr., Kyle TX, the employer failed to ensure the employees were protected from a free fall greater than six feet. The employees were installing asphalt shingles and their fall arrest system was not installed to prevent them from free falling more than six feet to the lower or swinging into the cement wall.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about August 27, 2015, at 5020 Cromwell Dr., Kyle TX, the employer failed to ensure that each employee that was installing asphalt shingles were trained to recognize fall hazards and how to effectively use the fall protection equipment. The employees were exposed to a free fall hazard greater than six feet. They were working on a roof up to approximately 34 feet high.
