OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazard;     a) On or about 08/28/15 in the Beam Saw area; employer failed to protect employees from the fire and explosion hazard when two dust collectors for wood processing equipment, such as but not limited to, the Beam saw, CNC router, Edge bander, were located inside and without any fire prevention or suppression systems.      ABATEMENT DOCUMENTATION REQUIRED     National Fire Protection Association standard, NFPA 664 - 2012 Edition, Prevention of Fires and Explosions in Wood Processing and Woodworking Facilities, Section 8.2.2.5.1.4 requires; Dust collectors shall be located in accordance with one of the following:     Among other methods, one feasible and acceptable abatement method to correct this hazardous condition would be, such as but not limited to;     When dust collectors are located indoors;     1) Equip the dust collectors with a listed deflagration suppression system.   2) Equip the dust collectors with deflagration relief vents with relief pipes extending to safe areas outside of the building and verify the collectors met strength requirements of this standard.   3) Equip the dust collectors with deflagration relief vents exhausting through listed flame-quenching devices and verify the collectors met the strength requirements of this standard.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about 08/28/15 in the Beam Saw area; employer failed to protect employees from unexpected contact with a moving saw blade and product carriage when a procedure was not documented and utilized, to control hazardous energy (electric power and kinetic energy of rotating saw blade) during service and maintenance activities of the Beam Saw, such as but not limited to saw blade changing, cleaning dust accumulation, greasing the carriage rails of the beam saw.         b) On or about 08/28/15 in the Edge Bander area; employer failed to protect employee from unexpected contact with moving parts of the Edge Bander when a procedure was not documented and utilized to control hazardous energy (pneumatic and electric power) during service and maintenance activities, such as but not limited to, unjamming, changing tooling, cleaning the internal parts of the edge bander.
