29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe, was not taken out of service until it had been restored to safe operating condition: On or about August 27, 2015, in the facility, powered industrial truck(s) with defect or in any way unsafe had not been withdrawn from service until responded to safe operating condition as follows: a. Toyota "R13" Model Number 7FGCU25 Serial Number 87984 was emitting 4.6% carbon monoxide at full throttle. b. Toyota "R10" Model Number 7FGCU25 Serial Number 91627 was emitting 2.2% carbon monoxide at idle. c. Toyota "R02" Model Number 8FGCU25 Serial Number 45613 was emitting 1.5% carbon monoxide at idle. d. Toyota Model Number 7FGCU25 Serial Number 60452 was emitting 4.33% carbon monoxide at idle. e. Toyota "R11" Model Number 7FGCU25 Serial Number 85019 was emitting 2.23% carbon monoxide at idle. f.
