29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm):    a) On or about August 31, 2015- job site at 129 Ridge Road, Douglaston, NY 11363    the employer permitted employee's to work approximately 15 feet above the next lower level on a scaffold that was not     fully planked.    WRITTEN ABATEMENT CERTIFICATION IS NOT REQUIRED.
