29 CFR 1910.22(a)(3): Floors, working places, and passageways were not kept free from protruding nails, splnters, holes, or loose boards. Facility, Test Area: The edges of two 6 foot X 4 foot rubber flooring panels were protruding from the floor causing a tripping hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws. Workshop: An orang and black striped extension cord was not provided with a strain relief fittings. The outer sheathing of the extension cord was pulled back and the three inner conductors were exposed to physical damage.
