29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. On northeast section of the roof of the Countryside Recreation Center where work was being done for the installation of a heating, ventilation and air conditioning unit, an employee was not protected from falling through a hole (approximately 4ft by 8ft), exposing the employee to a 30 foot fall hazard, on or about 8/20/2015.
