29 CFR 1910.134(h)(3)(iii): Air and oxygen cylinders of self-contained breathing apparatus were not maintained in a fully charged state: On or about August 12, 2015, and at times prior thereto, for employees using MSA PremAire Supplied Air Respirators during operations such as, but not limited to, chlorine gas cylinder change-out, the employer did not ensure that each supplied air respirators emergency escape cylinder was maintained in a fully charged state. Employees were exposed to hazards associated with exposure to chlorine gas.
