29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:     On or about August 21, 2015, the employer did not ensure energy control procedures were developed which clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy and the means to enforce compliance. The employer did not ensure employees were utilizing the procedures for the safe application, usage, and removal of energy controls during die changes, die work, and other servicing and maintenance activities. Employees were exposed to electrical, pneumatic, hydraulic, kinetic, etc. energy sources.

An employee received a three finger amputation while operating a full revolution mechanical power press equipped with Possum pullbacks.

29 CFR 1910.217(c)(3)(i)(c): 29 CFR 1910.217(c)(3)(i)(c): The employer did not provide and ensure the point of operation device prevented the operator from inadvertently reaching into the point of operation at all times:  On or about August 21, 2015, the employer did not ensure the point of operation device prevented the operator from inadvertently reaching into the point of operation. During the assembly of part 726 inner and outer, the operator was able to get left hand into the point of operation resulting in an amputation of left pointer, middle, and ring fingers.
