29 CFR 1910.36(d)(1): Employees were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    (a) Warehouse:  On or about 8/31/2015, the emergency exit door at the rear of warehouse was secured from the inside using a hasp bolt that would need to be unlocked in order to evacuate the building.     Quarles II, Inc. was previously cited for a violation of this occupational safety and health standard, 1910.36(d)(1), which was contained in OSHA inspection number 844363, citation number 001, item number 02 and was affirmed as a final order on 5/29/2015, with respect to a workplace located at 666 Park East Drive, Woonsocket, RI 02895.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:    (a) Warehouse - Overhead door for cardboard trash container:  On or about 8/31/2015, access to the portable fire extinguisher mounted near the overhead door was blocked by cardboard boxes and debris.    Quarles II, Inc. was previously cited for a violation of this occupational safety and health standard, 1910.157(c)(1), which was contained in OSHA inspection number 844363, citation number 001, item number 03 and was affirmed as a final order on 5/29/2013, with respect to a workplace located at 666 Park East Drive, Woonsocket, RI 02895.
