29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition when it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.    a) Carmelite Monastery, Worksite - on or about August 28, 2015, employees are performing mason tendering work by rigging buckets of mixed mortar to a scaffold hoist, which weighs approximately 22.5 pounds when one quarter full, without wearing any means of head protection.    b) Carmelite Monastery, Worksite - on or about August 28, 2015, employees are performing mason tendering work by filling buckets of mortar, which contains abrasive and corrosive materials, from a powered mixer without wearing any means of eye protection
