29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less: (a) Born Builders on or about August 25, 2015, at the address of 15121 Mason Rd. Vermillion, OH: The employer failed to ensure the components of the ladder jack scaffolding system used by employees onsite, were not loaded in excess of their individual rated capacities. Two employees operating from the ladder jack scaffolding (exceeding the minimum component rating of 250 lbs.) were exposed to scaffold failure and ultimately a fall of approximately 10 ft. to the ground below.

Vermillion, OH: The employer failed to ensure the components of the ladder jack scaffolding system used by employees onsite, were inspected by a competent person prior to each shift for visual defects such as cracks in the fiberglass ladder. Two employees operating from the ladder jack scaffolding were exposed to scaffold failure and ultimately a fall of approximately 10 ft. to the ground below.
