29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Production Floor - Building 3 - The employer did not provide adequate guarding for the employees who operate the hydraulic press, Press-1. An employee was severely injured when operating Press-1, the press continued operating when the employee's hands entered the danger zone on or about August 26, 2015.  b) Maintenance Room - Employee was not protected from the hazards created by an unguarded rotating chuck on the Clausing drill press, Model #1649, and Serial #116768. This violation exposed the employee to rotating parts and flying parts from material or bit, when in operation. This violation was observed on or about November 5, 2015.    The company Ideal Products of America, LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 977345, citation number 1, item number 1 and was affirmed as a final order on December 19, 2014, with respect to a workplace located at 3239 Phoenixville Pike, Malvern, PA.
