29 CFR 1926.96: The employer did not ensure employees wear proper footwear which meets the requirements and specifications in American National Standard for Men's Safety-Toe Footwear, Z41.1-1967: On or about 28 August, 2015, at the above addressed jobsite, two employees were working inside a new home under construction without the use of proper footwear, exposing the employees to the hazards of being struck by falling tools, materials and or stepping on exposed nails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about 28 August, 2015 at the above addressed jobsite, an employee was stripping the formwork of a new residential home under construction, without utilizing fall protection, exposing the employee to a fall hazard of approximately 14 feet.
