29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks was not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: On September 01, 2015, employees in the molding department were working near propane portable tanks that were not secured. This condition exposed employees to explosion hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one-eighth inch: Electrical department: On September 1, 2015, the work rest of the 6-inch Delta bench grinder (Model #GR150, 3600 RPMS, 60Hz) exceeded the one-eight inch maximum adjustment. The distance between the work rest and wheel measured approximately one-inch. This condition exposed employees to stuck-by hazards.
