OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from impalement hazards:    a) Multiple bolts extended approximately 3 inches vertical from the concrete along the west side of the building. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)     a) East side of structure to access the higher roof level:  The unsecured fiberglass extension ladder did not reach the higher of the roof levels and there was a gap of approximately 1 to 2 feet between the top of the ladder and the roof being accessed by workers.    b) West side of structure to access the lower roof level: The unsecured aluminum extension ladder extended only one rung above the lower of the roof levels being accessed by workers involved in wood sheathing.    Note: Abatement certification is NOT required for this item.  The ladders were observed corrected during the inspection

29 CFR 1926.501(b)(2)(i): Each employee who is constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.    a) On or about August 31, 2015, and at times prior thereto, two workers installing wood sheathing on a nearly flat roof were not protected from fall hazards.  The lower level of the roof was 14 to 15 feet high and the upper level was 17 to 18 feet high to the ground, concrete, equipment and materials below.    Note 1: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.    Evans Framing, LLC was previously cited for a violation of this was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 855083 citation number 1, item number 2 and was affirmed as a final order on May 20, 2013, with respect to a workplace located at the Corner of Franklin Road and Orchard, Boise, ID 83706.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:    a) Workers engaged in wood framing had not received training to understand how to properly connect and use personal fall arrest systems during and sheathing of a low sloped roof.  Two workers were engaged in leading edge work and aligning "tube" trusses.  When they did attempt to anchor, they were unfamiliar with how to do it properly to allow them to do their work and prevent them from hitting the ground in the event of a fall.    NOTE: Abatement certification IS REQUIRED for this item.
