29 CFR 1910.269(r)(1)(ii): There shall be a second line-clearance tree trimmer within normal (that is, unassisted) voice communication under any of the following conditions: If a line-clearance tree trimmer is to approach more closely than 3.05 meters (10 feet) to any conductor or electric apparatus energized at more than 750 volts. On or about August 30, 2015, during tree trimming line and clearance activities, at a jobsite located at 2123 West 106th, Cleveland, Ohio 44102, the employer failed to protect an employee from an electrocution hazard. The employee was performing work within the maximum allowable distance of electrical components, which were energized with up to 4.34 KV Delta, without a second line-clearance tree trimmer within normal voice communication range.
