29 CFR 1926.1053(b)(12): Ladders used where the employee or the ladder could contact exposed energized electrical equipment did not have nonconductive side rails:  On or about 8/22/2015, 58 Cook Ave.  a) The employee was carrying a 12' section of an aluminum extension ladder to the upper level. He contacted the energized power lines.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about 8/22/2015, 58 Cook Ave.  a) Employees using ladders were not provided training as required by the standard.
