29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Sewer and water service, 1011 South Main Street, Rice Lake, WI ; workers were unprotected while working in an excavation outside of a trench box and did not have adequate cave-in protection. The excavation was approximately eleven feet deep with an excavation face that was greater than 65 degrees.
