29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about 08/25/2015, at the jobsite at 1222 Allen Ave, St Louis, MO, employees were installing plywood pieces to bell tower using a Ryobi 18 volt Impact Driver, Model # P236A, and were not wearing safety glasses.
