29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 08/20/2015, at the Super Abrasive Manufacturing area, Department 423, an employee was engaged in training operations using the Mazak Mill Turn Machine, Model #INTi200S, Serial #228438, 120 Volts. Employee was attempting to manually move the "Jaw" (C-Axis) to the horizontal position so that a blank part could be more easily installed. The jaw clamp was in the open position and was used to hold the part in pace once installed. Employee was exposed to amputation hazards when the jaw became energized, which resulted in the loss of the tips of an index and middle finger from the closing of the clamp.
