OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that Scissor Lift work platforms had defective slope limit alarms, and missing safety pins for guardrail systems. a). On or about September 2, 2015, employees were operating a Genie lift brand elevated work platform, Model #GS3232, serial #848150RA with an inoperative slope alarm or limiter, exposing employees to a 30 foot fall hazard. b). The top rail of the guardrails on the Genie Model GS3232, Serial #848150RA, lift did not have the manufacturer approved safety pins installed, exposing the employees to a 30 foot fall hazard.

29 CFR 1926.352(d): Suitable fire extinguishing equipment shall be immediately available in the work area and shall be maintained in a state of readiness for instant use. a). On or about September 23, 2015 Employees using Oxy/Acetylene torches to cut steel I-beams about 25 feet above the floor, and were not protected from fire or explosion hazards because no suitable fire extinguishing equipment was readily available to extinguish accidental fires.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) On or abut September 2, 2015, a temporary flexible cord used to energize portable hand grinders and Saws, and were not protected from electrical shock hazards by a path to ground that was permanent and continuous because the ground pin on the cord had been removed or broken off exposing employees to 110 Volt electrical shock hazards.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: 29 CFR 1926.502(d)(15)(i): as part of a complete personal fall arrest system which maintains a safety factor of at least two; a). On or about September 2, 2015 employees working at heights about 30 feet off the ground to remove existing steel I beams from a demolished passenger ramp, did not have the proper anchor points to attach personal fall protection systems while working at heights, exposing employees to 30 foot fall hazards.
