29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Employees were exposed to crushing and struck by hazards because to increase the reach of the Tusk 500-PG16 forklift the employer added a homemade fork extension to the right fork. To attach this extension the employer created a hole in the forklift fork and written approval from the manufacturer was not acquired before the modification was made.

29 CFR 1910.178(a)(5): The user did not request, for trucks equipped with front-end attachments other than factory installed attachments, that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. Employees were exposed to crushing and struck by hazards because the employer added a homemade front-end attachment to the Tusk 500-PG16 forklift which extended the reach of the lift and changed the center of gravity for the forklift. The employer did not have the truck's capacity evaluated with the attachment installed.
