29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Norwalk Concrete Industries, Inc. at the worksite located at 191 Woodlawn Ave. in Norwalk, OH: On August 27, 2015, the employer did not ensure that employees were protected from pinch point hazards created when the core in the 8 ft. x 8 ft. molding machine is opened/inflated. An employee sustained partial finger amputation injuries when caught between the core and mold frame when the core was inflated.
