29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph:      a)  On or about September 2, 2015, employees were exposed to electrical hazards where electrical panels did not have covers exposing them to accidental contact with live parts.     b)  On or about September 2, 2015, employees were exposed to electrical hazards where electrical boxes did not have covers exposing them to accidental contact with live parts.       In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
