29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) Lemo # 4 machine: An employee was exposed to an amputation hazard from the Lemo # 4 plastic bag printing machine that did not have a guard in place to prevent the operator from having any part of his body in the danger zone. On 8/25/2015, an employee sustained injuries to three fingers on his right hand from the Lemo # 4 plastic bag printing machine. The Omega Plastic Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3):, which was contained in OSHA inspection number 1049212, citation number 1, item number 1 and was affirmed as a final order on 10/2/2015, with respect to a workplace located at Page and Schuyler Avenue, Lyndhurst, NJ 07071.
