29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    On September 1, 2015, at the above addressed jobsite, an employee was working on the roof of a residential home using a pick fork around an energized power line exposing the employee to electrocution hazards.       Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    On September 1, 2015 at the above address employees were performing roofing work on a residential home. Employees were not utilizing fall protection and were exposed to a fall hazard of 12.5 feet in the back of the home and approximately 20 feet on the second story of the home.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On September 1, 2015, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three feet above the upper landing surface, thereby exposing employees to falls.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
