The employer had not ensured that subcontractor employees were protected from falls of 13 feet, 4 inches through the first floor open and partially guarded floor hole to the basement below. Violation observed on or about 10/15/2015. b) 23 Goodhart Drive, Second Floor Landing Around the Stairwell, Livingston, NJ. The employer had not ensured that subcontractor employees were protected from falls of up to 10 feet, 4 inches from the second floor landing to the stairs and first floor below as the top guardrails around the stair opening were only 34 to 35 inches in height and they were lacking a midrail. Violation observed on or about 10/15/2015.

The employer had not guarded the tops of 25 pieces of reinforcing steel projecting up approximately 17 inches from the ground in the general area where sill plate installation was occurring and subcontractor employees were walking. Violation observed on or about 08/31/2015.
