29 CFR 1926.404(b)(1)(ii): On this construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. (a) Worksite located at Flex Spaces at Kearny Point, 78 John Miller Way, Kearny, NJ. Employees were exposed to multiple instances of extension cords being plugged directly into the building's permanent wiring without GFCI protection being used. Violation observed on or about 9/2/2015.

The extension cord was being utilized to charge a scissor lift. (b) Worksite located at Flex Spaces at Kearny Point, 78 John Miller Way, Kearny, NJ. Employees were exposed to a scissor lift with a broken strain relief on its charging cord. The scissor lift was being charged at the time of inspection. Violations were observed on or about 9/2/2015.
