29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    (a) Colorado Structures, Inc. at 1232 S Hover Street, Longmont, CO: On and preceding 8/31/15, the employer did not initiate and maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Employees and subcontractor employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center.  On 8/31/15, a subcontractor employee prepping the steel decking for installation of insulation and membrane, fell through an approximate 3 foot by 5.5 foot hole in the decking.  Colorado Structures Inc did not determine the presence of the floor hole through an inspection of the jobsite and correct the hazard.  This condition exposed employees and subcontractor employees to an approximate fifteen foot fall hazard.    (b) Colorado Structures, Inc. at 1232 S Hover Street, Longmont, CO:  On and preceding 8/31/15 the employer did not initiate and maintain a safety program which provided for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Employees and subcontractor employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center.  Employees and subcontractor employees worked in proximity to floor holes that were not covered, secured, or marked.  Colorado Structures Inc did not determine the presence of uncovered, unsecured, or unmarked floor holes and correct the hazards.  This condition exposed employees and subcontractor employees to approximate fifteen foot fall hazards.

29 CFR 1926.502(i)(3): Employer did not ensure that all covers were secured when installed so as to prevent accidental displacement by the wind, equipment, or employees:    (a) Colorado Structures, Inc. at 1232 S Hover Street, Longmont, CO: On and preceding 8/31/15 the employer did not ensure that all covers were secured when installed so as to prevent accidental displacement by the wind, equipment, or employees.  Employees and subcontractor employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center.  The employees and subcontractor employees worked in proximity to an approximate 3 foot by 5.5 foot floor hole in the decking.  The decking hole had been covered by a four foot by eight foot sheet of OSB.  The sheet of OSB that had covered the hole was not secured to the decking.  This condition exposed the employees and subcontractor employees to an approximate fifteen foot fall hazard.    (b) Colorado Structures, Inc. at 1232 S Hover Street, Longmont, CO: On and preceding 8/31/15 the employer did not ensure that all covers were secured when installed so as to prevent accidental displacement by the wind, equipment, or employees.  Employees and subcontractor employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center.  The employees and subcontractor employees worked in proximity to a roof hatch covered by a pallet missing two slats.  The pallet that covered the hole was not secured.  This condition exposed the employees and subcontractor employees to an approximate fifteen foot fall hazard.

Employees and subcontractor employees performed roofing activities on the Shops E building at the Village at Twin Peaks shopping center. Employees and subcontractors worked in proximity to holes in decking for ducting for rooftop air handling units (RTU's).  Dimensions of paired holes for each RTU were fourteen inches by twenty inches and eighteen inches by nineteen inches.  The sheets of four foot by eight foot OSB used as covers for five of the paired floor holes were not marked to warn of the hazard.  This condition exposed employees and subcontractor employees to an approximate fifteen foot fall hazard.
