(a) Work site was located at Flex Spaces at Kearny Point, 78 John Miller Way, Kearny, NJ. Employees had stored oxygen and acetylene bottles vertically with no secure measures right outside of the elevator door. Violation observed on or about 9/2/2015.

29 CFR 1926.350(a)(10): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour. (a) Work site was located at Flex Spaces at Kearny Point, 78 John Miller Way, Kearny, NJ. Employees were storing oxygen and acetylene bottles in close proximity to each other with no barrier between them. Violation was observed on or about 9/2/2015.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level. (a) Work site was located at Flex Spaces at Kearny Point, 78 John Miller Way, Kearny, NJ. Two employees disassembling the freight elevator on the fourth floor had two extension ladders improperly set up to access the top of the hoist beam. The ladders were not secured at the top so that the ladders would not deflect or be displaced due to movement of the elevator floor. Violation was observed on or about 9/2/2015.
