29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). a- Garden State Tile, Dayton, NJ, Throughout Warehouse - Training and Written Certification was not done for all employees operating Forklift Trucks. Training to include, but not limited to: -Truck safety related topics and workplace related topics; -Evaluation of the operator's performance in the workplace. Certification is to include, but not be limited to, the name of the operator, the date of the training or the evaluation, the name of the person providing the training or evaluation, etc. Condition noted on or about 9/8/2015.

29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, holes, or loose boards: a/ Garden State Tile/Dayton, NJ Site - Outside Warehouse Bay Door near outside dumpster: The loading dock was not maintained. Large cracks and missing chunks of concrete were not covered, sealed and/or other approved means provided where industrial trucks travel and pedestrians walk. Observed on/or about 9/8/2015.
