29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about September 3, 2015, in the Machine Shop, the employer failed to ensure that the point of operation guard on a HEM Saw (horizontal band saw, Model Cyclone, Serial number 977207) was adjusted to the point of operation. The employee was cutting a metal for a skid. Custom Tower, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 890805, citation number 1, item number 1 and was affirmed as a final order on March 23, 2013, with respect to a workplace located at 402 Facile Road, Scott, LA.
