29 CFR 1926.651(i)(3): Sidewalks, pavements and appurtenant structure shall not be undermined unless a support system or another method of protection is provided to protect employees from the possible collapse of such structures. a. On or about September 4, 2015 - at the above addressed jobsite, employees were performing water line installation in a trench, ten (10) feet three (3) inches in depth, that undermined the parking lot, thereby exposing employees to the hazards associated with pavement collapse.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about September 4, 2015 - at the above addressed jobsite, employees performing water line installation in a trench, ten (10) feet three (3) inches in depth, that was not provided with cave in protection, thereby exposing employees to cave in hazards.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. a. On or about September 4, 2015 - at the above addressed jobsite, the portable ladder used to access and egress the trench did not extend above the landing surface, thereby exposing the employees to falls of six (6) feet seven (7) inches.
