29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 712-718 NORTH 3RD STREET, HARRISBURG PA, ROOFTOP - An employee performing roofing work on the third story roof was not protected by any means of fall protection, thereby exposing the employee to fall hazards of approximately thirty-three (33) feet to the ground below, on or about September 3, 2015. (b) 712-718 NORTH 3RD STREET, HARRISBURG PA, ROOFTOP - An employee cleaning the gutters was not protected by any means of fall protection, thereby exposing the employee to fall hazards of approximately thirty-three (33) feet to the ground below, on or about September 3, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) 712 - 718 NORTH 3RD STREET, HARRISBURG PA, ROOFTOP - Employees performing work on the third story roof used a twenty (20) foot extension ladder to gain access to the roof, and the ladder rungs only extended approximately thirty inches past the upper landing surface, thereby exposing the employee to fall hazards of approximately 7.5 feet to the surface below, on or about September 3, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
