29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or about September 3, 2015 the employer failed to develop and implement a written respiratory protection program that covered employees working in the woodshop that are exposed to respiratory lead hazards and are required by the employer to wear tight fitting respirators while performing activities that can generate airborne lead such as sanding, cutting and blasting.

29 CFR 1910.1025(d)(1)(ii): Full shift (for at least seven -7 continuous hours) personal samples for lead were not collected including at least one sample for each shift for each job classification in each work area: On or about September 3, 2015, throughout The Bank Architectural Antiques Facility, the employer failed to ensure that sampling collected was full shift, at least seven (7) continuous hours, for airborne lead exposures to determine if employees were exposed to lead at or above the action level of 30 ug/m3.
